Personal Training and Group Fitness Policies and Procedures
BY CHOOSING TO TRAIN WITH KI FITNESS YOU AGREE TO OBIDE BY THE FOLLOWING TERMS, CONDITIONS AND POLICIES. KI FITNESS RESERVES THE RIGHT TO TERMINATE ANY AGREEMENT SHOULD THESE TERMS, CONDITIONS AND POLICIES NOT BE ADHERED TO.

The purpose of these policies is to manage expectations and communication between you, the client, and the coach. It is to manage the safety of all patrons and create equality.
SCHEDULING YOUR SESSION
All scheduling, rescheduling or cancellation of Personal Training Services should be done through Sean Carville by calling 0402 883 366 or emailing firstname.lastname@example.org.
Personal Training Sessions are required to be paid for in advance to secure your place. This can be done via cash payment or direct deposit to:

Group fitness and direct debit membership packages incur the following:
- NO CONTRACT
- Set up with Ezypay, require completion of member application form and photo identification.
- Once off customer load fee of $2.20 (incl. GST) applies to all direct debit packages.
- Customer failed payment fee of $9.79 (incl. GST) should nominated direct debit account be in arears.
- Cancelation of direct debit requires a minimum of 2 weeks’ notice and may incur dues owed in that time.
DIRECT DEBIT PAYMENT DECLARATION
I agree that the liability for the agreed payment submitted with my application shall continue as agreed notwithstanding my frequency of attendance at the facility or that I cease to attend altogether. I agree & understand that direct debit memberships are continuous & that unless I inform otherwise, my membership AND payments shall continue indefinitely.
I shall not be entitled to any refunds for any reason.
As a member, I shall cease to have rights to the centre, at its discretion, if payments are not received by their due date. As a member I shall pay all costs of the collection of any funds due including any debt collectors, legal fees or other costs payable by my defaults. I acknowledge that I have read and understand all of the above provisions as well as the Ki Fitness Personal & Group Fitness Training Policies & Procedures (found at http://www.kifitness.net.au/policies---procedures.html) and acknowledge that this is a legally binding contract and that I observe to be bound by it.
If you need to cancel or reschedule a personal training services, please call Sean Carville on 0402 883 366. If he is not in, be sure to leave him a message. He will check his availability and get back to you with confirmation of the rescheduled date and time. If it is a last minute emergency, please call the emergency in to Sean directly. 12 hours’ notice is required for a cancellation or rescheduling in order to receive credit for the session. Failure to cancel within this time frame or failure to show up for a session will result in the client being charged for the session. Exceptions will only be made in the case of a medical emergency accompanied by a doctor’s note.
Clients are expected to begin their session at the start time of the scheduled appointment. A late start time does not entitle a client to a session longer than the scheduled appointment.
REFUNDS, FREEZES & TRANSFERS
Pre‐paid Personal Training sessions are non‐refundable and non‐transferable. If a client becomes ill or injured and has been advised by a physician to temporarily discontinue training, the Trainer will freeze any remaining sessions for a period of up to 3 months from the original purchase date.
POLICY CHANGES & RATE INCREASES
Ki Fitness reserves the right to review and adjust the Personal & Group Training Policies and fee schedule at least annually and will notify clients accordingly.
ANTI‐DISCRIMINATION & HARASSMENT
Ki Fitness is committed to ensuring that the working/training environment is free from discrimination and harassment. Discrimination and harassment will not be tolerated under any circumstance. Disciplinary action will be taken against any employee (or agent) and/or client who breaches the policy.
Discrimination and harassment occur when a person is discriminated against or harassed in the workplace and in certain areas of public life:
· because of their race, colour, descent or national or ethnic origin, as defined under the Racial Discrimination Act 1975
· or because of their sex, marital status, pregnancy as defined under the Sex Discrimination Act 1984
· or because of a disability as defined under the Disability Discrimination Act 1992
· or because of age as defined under the Age Discrimination Act 2004
· and some grounds under the Australian Human Rights Commission Act 1986.
RELEASE FROM LIABILITY / INDEMNITY
I hereby declare that the information and particulars given to Ki Fitness are true & correct.
I agree, being aware of my own health and physical condition, and having knowledge that my participation in any exercise program may be injurious to my health, I am voluntarily participating in physical activity with Ki Fitness. Having such knowledge, I hereby release Ki Fitness, their representatives, agents, and successors from liability for accidental injury or illness, which I may incur as a result of participating in the said physical activity. I hereby assume all risks connected therewith and consent to participate in said program.
I agree to disclose any physical limitations, disabilities, ailments, or impairments that may affect my ability to participate in said fitness program.
I agree to terms & conditions of policies and procedures.
